USA Hockey Requirements
Must be completed before participating in any team activities

1. Register as a member of USA Hockey. (Cost is $46 plus any affiliate fees if applicable). Click here to register.  Once you have registered click the link above to send FYHA your USAH Confirmation number.
2. If you have any questions about your status please use USA Hockey's online tool before asking Nate or Jeff.  You have access to all your CEP information including SafeSport status, screening status, Level Certification expiration, modules needed, etc. CLICK HERE TO CHECK YOUR USA HOCKEY CEP INFORMATION
3. Complete USA Hockey's mandatory background screening every two years.  If you completed this process during the 2022-23 season you do NOT need it for 2023-24.  If you are unsure please reach out to FYHA Admin.  USA Hockey has already entered your self-registration ID number on the first page of the screen. You do not need to change this number.  If your page does not auto-populate with the ID copy and paste this one:  35615801.  Click here to complete the background screening.
4. Complete the mandatory Safe Sport Training online.  This is required every year and a coach cannot be added to a roster until completed. Click here to start the SafeSport process. Do not email your SafeSport certificate to FYHA Staff.
5. Complete the online age-specific module(s) for the age/level you are coaching. This must be completed PRIOR to participating in any team activities and a coach cannot be added to a roster until completed. Click here to register for the modules.
6. Find, register and attend the required USA Hockey Coaching Education Program certification clinic. The Cost is $55. You can only attend one clinic per season and all coaches start at Level 1. You can take clinics from April 1-Dec. 31 each season. Click here to locate and register for a clinic.
7. FOR TRAVEL COACHES ONLY:  If you will be receiving a stipend for coaching with the Freeze, electronically sign and submit your IRS Form W9.  Click here to submit your IRS Form W9.
8. FOR TRAVEL COACHES ONLY:  Electronically sign and submit your 2023-24 FYHA Coaching Agreement that you will receive from Hockey Director Nate Metcalf via email.
